This is lit by the Nikon SB-22s in A1 mode, set for 90 degree bounce flash (the flash is tilted straight up and fired at the ceiling). Note the lack of harsh shadows and better-defined background. This is due to the diffuse lighting effects produced by bouncing the flash off the white ceiling.
